REX PiezoImplant certified for narrow ridges

Measured in millimeters

A 3.7mm ridge is not a dead end.

When a tooth has been missing for years, the jawbone narrows. Below roughly five millimeters of width, a standard round implant no longer fits, and most offices will tell you the same thing: graft first, wait months, then come back.

There is a second route. REX PiezoImplants are wedge shaped rather than round, only 1.8mm thick, and placed with piezoelectric instruments that shape bone without a drill. They are designed to go into a narrow ridge as it already is.

Find out if this applies to you

One honest caveat. A wedge implant solves a ridge that is too narrow. It does not solve a ridge that is too short, or one sitting too close to the sinus. Those still call for grafting or a sinus lift, both of which we do here. A 3D scan is what tells us which situation you are actually in, and that is the first thing we do.

STANDARD IMPLANT 5.0mm + ridge width needed WEDGE IMPLANT 1.8mm thick 3.7mm no graft required

Cross section, not to scale. Figures reflect a seven year multicenter study of wedge shaped implants placed in horizontally narrowed ridges without bone augmentation. Individual candidacy is determined by 3D scan.

What comes through our door

The cases that get referred out elsewhere

Many general offices place straightforward implants and send anything complicated to a specialist across town. We handle the diagnosis, the surgery, and the final restoration under one roof on North Street.

Told you are not a candidate

Bone loss is the single most common reason a patient is turned away. Between wedge implants for narrow ridges, bone augmentation, and sinus lifts, there is usually a route. It starts with a scan, not an opinion.

Full arch and All-on-X

Failing teeth or dentures that never sat right. A fixed arch anchored on implants removes the adhesive, the slipping, and the food you have quietly stopped ordering.

Second opinions on someone else's plan

Bring the quote and the scan. We will tell you plainly whether the plan makes sense, including when the answer is that the other office got it right and you should stay put.

Failed or failing implants

Implants placed elsewhere that have loosened, become infected, or lost bone around them. Assessment, removal where needed, and a plan to restore the site properly.

Sedation for high dental anxiety

If avoidance is the reason this has waited years, sedation makes the appointment something you can get through. Tell the front desk when you call so we plan for it.

Complex medical histories

Diabetes, blood thinners, bisphosphonates, prior radiation. These change the plan rather than end it, and they are worked through before anything is scheduled.

The dentist doing the work

Implants are not a side service here

The goal was never to fix your teeth for today. It is to give you something that is still working in twenty years.

Plenty of dentists added implants to the menu after a weekend course. Dr. Schoengold started placing them in 1996 and has spent the three decades since on the cases that are not straightforward: severe resorption, full arch reconstruction, medically complex patients, and people who arrived having already been told no somewhere else.

  • Diagnosis and surgery and restoration in one office. No handoff to a specialist across town, no gap where nobody owns the outcome.
  • Conservative by default. A natural tooth that can be saved gets saved. Implants get recommended when they are genuinely the better option.
  • Plain language. You get shown your own scan and told what it means before any plan is discussed.
  • Written plan before anything begins. Scope, sequence, and cost in writing, so nothing about the number moves later.

Treatment options

One tooth, several, or the whole arch

Different problems, different hardware. Which one applies is decided from the scan, not from a brochure.

01 / SINGLE

Single tooth implant

Replaces one missing tooth on its own root. Nothing is ground down on either side, which is the main reason it beats a traditional bridge long term.

02 / MULTIPLE

Implant supported bridge

Two or more implants carry a bridge across a gap. The load sits on titanium rather than on the neighboring teeth.

03 / FULL ARCH

Full arch, All-on-X

A fixed arch on four to six implants. It does not come out at night and it does not need adhesive.

04 / DENTURE

Implant overdenture

A removable denture that snaps onto implants instead of resting on gum. The lower cost route to stability.

What actually happens

From first phone call to final tooth

Timelines below are typical. Grafting, healing speed, and your own medical history move them.

STEP 01

Consultation and 3D scan

A CBCT scan shows the bone you actually have. You see it on screen and get told what it means.

One visit
STEP 02

Written plan and cost

Scope, sequence, and the full number in writing. Insurance benefits checked before you decide anything.

Within a few days
STEP 03

Placement

Position planned digitally in advance. Local anesthesia as standard, sedation if you want it.

One appointment
STEP 04

Healing and integration

Bone fuses to the implant surface. You are checked along the way, with a temporary in place where needed.

Roughly 3 to 6 months
STEP 05

Final restoration

The crown, bridge, or arch is fitted and adjusted until the bite is right and it disappears in the mirror.

Questions we get on North Street

Another dentist said I do not have enough bone. Is that final?

Often it is not. There are three separate situations that all get described the same way. If the ridge is too narrow, a wedge shaped implant may fit without any grafting. If the ridge is too short, grafting can rebuild it. If the upper back jaw has thinned toward the sinus, a sinus lift creates the height. What you need is a 3D scan to establish which one you are dealing with, because they have different answers.

What does a dental implant cost in White Plains?

There is no single figure, because the honest answer depends on what your scan shows. Whether grafting is needed, how many teeth are involved, and which restoration goes on top all move the number substantially. What we can promise is the process: you get the full cost in writing before you commit to anything, your insurance benefits are checked first, and the number does not move afterwards. Monthly payment plans are available.

Will my insurance cover any of it?

Usually part of it. Few plans cover the implant fixture itself, but many cover the extraction, the graft, or the crown that sits on top. We check your specific benefits before treatment and tell you the real out of pocket number, not an estimate. Monthly payment plans are available for the balance.

Can I bring a treatment plan from another office for a second opinion?

Yes, and a good number of our patients arrive that way. Bring the plan, the quote, and the scan if you have it. You will get a straight assessment, including the times when the honest answer is that the original plan was sound and you should go ahead with it.

How long does the whole process take?

For a straightforward single implant, roughly three to six months from placement to final crown, most of which is healing rather than appointments. Grafting adds months on the front end. Full arch cases using immediate loading protocols can leave the office with fixed temporary teeth the same day as surgery.

Does it hurt?

Most patients report less discomfort than they expected, and frequently less than the extraction that preceded it. The procedure itself is done under local anesthesia. Afterwards, over the counter medication handles it for the majority of people.

I am genuinely frightened of the dentist. What are my options?

Say so when you call and it gets planned around. Sedation is available, and for a lot of people it is the reason a problem they have avoided for years finally gets dealt with. There is no lecture about why you waited.

Do implants last?

They are designed to be permanent, and with good hygiene and regular checkups many last decades. The crown on top may need replacing at some point, which is normal wear. What matters most for longevity is that the implant was planned and positioned properly in the first place, which is where the scan and the digital planning earn their keep.
